How Do I Make An Appointment?

Call us at (763) 571-4000 to make an appointment for all locations. If there is a location that you would prefer or desire please specify the location when scheduling your appointment. We will ask that you describe the reason for your visit so we can schedule you with the appropriate dermatologist and for the adequate amount of time. It is important to note that not all procedures are performed at all locations.

We are happy to schedule you with a provider of your choice. Please be prepared to provide your demographics (phone number, addresses, etc.) and current insurance information. Please note, we are unable to schedule and secure your appointment without this information.


How Do I Refill My Prescription?

Call your pharmacist to refill a prescription. The pharmacist will then call our office for approval.

Prescriptions can be approved only during office hours. Please allow 24-48 hours to approve a refill. If your refill request is denied you will need to contact the doctor.


How Do I Get to Your Office?

Please visit our Contact Us page to see a map and addresses of all of our locations.


What If I Cannot Make My Appointment?

If you are unable to make your appointment, please call 763-571-4000 as soon as possible to cancel and reschedule your appointment.


Do All Of The Offices Offer All Services?

For the locations of each office, please visit our locations page. PLEASE NOTE! Not all services are offered at each office. Make sure you know your physician so you can be sure to schedule your visit in the right place.


What Do I Need To Bring For My Appointment?

For your appointment, you will need to bring your picture ID, insurance card, and insurance co-payment.


May I Fill Out My Registration Forms Before I Come For My Visit?

NO SHOW POLICY

Should you need to cancel or change the date of your appointment, we require a 24-hour notice so that we may accommodate other patients.  Failure to show up for a scheduled appointment without 24-hour notice will be considered a “no-show”.  Two no-show appointments within 12 months will be subject to dismissal from the practice.  As a courtesy, appointment reminders are sent via email, text, or voice call.  If you do not receive a reminder call or message, the no-show policy will still apply.  If you are running late for your appointment, please call the clinic where your appointment is scheduled and your dermatologist will try to accommodate you into their schedule; however, this is not always possible and your appointment may need to be rescheduled.
